Plum Health’s team of Personal Support Workers and Rehabilitation Therapists is dedicated to delivering optimal personalized affordable care under the guidance of regulated health professionals. We are licensed by the Financial Services Regulatory Authority of Ontario (FSRA) which, together with our internal integrated systems, ensures minimum delays in approvals and payments.

We provide in-home care to support all your needs with adept and compassionate people who make a difference. Being at the forefront of emerging healthcare techniques and support services is important to us so that we can better serve you. We pride ourselves on staying current in our knowledge through continuous education.

Our Team

Hazel Wood

President & cORRDINATOR

Hazel is the President at Plum Health. She has been a health care manager and clinician in hospital and community settings working with adults, adolescents, and children with a variety of conditions. Her areas of clinical interest include traumatic brain injury, mental health, chronic pain, spinal cord injury, arthritis, and musculoskeletal conditions.

Hazel is a past board member of the Ontario Brain Injury Association (OBIA) and the Ontario Rehab Alliance (ORA). She is a member of the OSOT Auto Insurance Sector Team. She has done strategic consulting for a number of not-for-profit agencies including Bone and Joint Decade Canada. In September 2010, she was awarded the honour of international Ambassador for the Bone and Joint Decade.

Hazel enjoys sailing, skiing, rollerblading, reading murder mysteries and spending time with family and friends.
